Community health survey listening session

You are invited to attend an upcoming listening session organized by the Chase and Morris County Community Health Needs Assessment local planning team. This team recently finished up a community survey in both Morris and Chase counties that gathered information on the health needs in our counties. They will use the data gathered in the survey to discuss the findings and gather additional information during a community listening session in each county. I hope that you would be willing to attend or send someone to represent your organization/business as they need as much feedback and insight as they can gather.
